Abdominal ultrasound is an imaging test that uses high-frequency sound waves to form real-time images of the organs in the abdomen. It does not emit radiation, is noninvasive and, in most cases, is painless. The specialist passes a transducer over the skin of the abdomen (a conductive gel is applied) and the images are instantly viewed on the screen.

Organs of the digestive system that can be visualized

This test can evaluate the liver, gallbladder, bile ducts, pancreas, spleen, kidneys, and abdominal aorta. It also helps identify free fluid in the abdominal cavity and detect abnormal masses or growths. According to the American Academy of Radiology, abdominal ultrasound is the initial test of choice for evaluating pain in the upper right quadrant of the abdomen.

Digestive symptoms that warrant an abdominal ultrasound

Persistent or recurrent abdominal pain

When abdominal pain lasts more than a few days, recurs frequently, or is located in specific areas (such as the upper right side or the “pit of the stomach”), the doctor usually orders an imaging study to guide the diagnosis. Ultrasound can rule out or confirm structural causes such as gallstones, inflammation of the pancreas, or liver abnormalities.

Bloating and excessive gas

Bloating accompanied by gas, flatulence, or frequent belching can be caused by many causes. Although it is often functional, in other cases it is related to organ problems that can be seen by imaging, such as free fluid in the abdomen, a partial obstruction, or masses that compress the bowel. Ultrasound helps rule out these possibilities, and with that basis, the doctor can better direct the rest of the study.

Nausea, vomiting, and jaundice

Yellowing of the skin or eyes (jaundice), when it occurs along with nausea and vomiting, may suggest a blockage of the bile ducts or liver disease. In these conditions, abdominal ultrasound is usually the first test ordered to check the liver, gallbladder, and bile ducts and determine if additional tests are needed.

Gallstones and gallbladder problems

Gallstones are one of the most common causes of abdominal pain in adults. Ultrasound has a sensitivity of more than 95% to detect them, making it the study of first choice. It also allows you to see signs of inflammation of the gallbladder, thickening of the wall and dilation of the bile ducts.

Acute Pancreatitis: Initial Imaging Evaluation

Acute pancreatitis is an inflammation of the pancreas that can cause severe pain in the pit of the stomach, nausea, and fever. Abdominal ultrasound is routinely ordered at baseline to identify gallstones as a possible trigger, assess the size of the pancreas, and detect peripancreatic fluid. In severe pancreatitis, intestinal gas can make it difficult to visualize the pancreas, but ultrasound is still the recommended starting point. The American Gastroenterological Association indicates that abdominal ultrasound should be performed in all patients with acute pancreatitis on admission.

Diseases of the liver and bile ducts

Fatty liver, cirrhosis, chronic hepatitis, and bile duct dilation are conditions that ultrasound can suggest with good accuracy. Ultrasound assesses the size, texture, and echogenicity of the liver, and can also detect focal lesions that require follow-up or a subsequent biopsy.

Evaluation of abdominal fluid and masses

The presence of free fluid in the abdomen (ascites) and the finding of enlarged masses or nodes are things that ultrasound can easily detect. This information guides the doctor on whether a CT scan, biopsy, or colonoscopy is needed to complete the study.

Preparing for Abdominal Ultrasound

Pre-Test Indications

To achieve good quality images, fasting for six to eight hours is recommended. This helps reduce intestinal gas and improves visualization of the gallbladder, which usually contracts after eating. The doctor may order additional restrictions depending on the reason for the study.

What you may feel during the procedure

It is usually a well-tolerated procedure. You may feel mild pressure when the transducer slides over your abdomen, especially if there are tender areas. The exam usually takes 20 to 40 minutes, depending on the complexity of the test. It does not require sedation or a subsequent recovery time.

Advantages of abdominal ultrasound

Among its main advantages are that it does not use ionizing radiation, is widely available, has a lower cost compared to other studies, and allows you to see structures in real time. It can also evaluate blood flow if Doppler is added, which provides useful vascular information in certain conditions. The World Health Organization recognizes ultrasound as an essential diagnostic imaging tool in primary and specialized care settings.

When Ultrasound Isn’t Enough

Intestinal gas can make it difficult to see deep structures, such as the pancreas, in people with marked abdominal distention. It is also less accurate at evaluating the small intestine, retroperitoneum, and small lesions. In those situations, the doctor may order additional tests.

Other complementary studies: CT scan, endoscopy and blood tests

Computed tomography offers more anatomical detail and is essential in severe pancreatitis, retroperitoneal masses or acute abdominal conditions. Upper gastrointestinal endoscopy allows direct observation of the esophagus, stomach and duodenum, and colonoscopy evaluates the colon. Blood tests provide data on liver function, markers of pancreatic inflammation and other parameters that complement the image. No study replaces another: the specialist defines which combination is the most appropriate according to the clinical picture. According to the American College of Gastroenterology, integrating imaging studies with the laboratory improves diagnostic accuracy in digestive diseases.

There are symptoms that should not be left for a scheduled consultation. If abdominal pain is severe, appears suddenly, or is accompanied by high fever, persistent vomiting, digestive bleeding, unintentional weight loss, or jaundice, medical attention should be sought immediately. These conditions may correspond to severe acute pancreatitis, biliary obstruction, visceral perforation, or other emergencies that require urgent evaluation and, often, immediate imaging studies.

Does abdominal ultrasound detect all digestive problems?

No. Ultrasound is very useful for evaluating solid organs such as the liver, pancreas and gallbladder, but it has limitations in studying the intestine, esophagus and very small lesions. How long does it take to get the results?
